How to file a SuperBill (Out of Network)
Here are some guidelines to help you file the “superbill” with your insurance company:
Blue Sky Family Counseling will provide you with a super bill through your secure portal or upon request.
1. Call your health insurance company prior to your first session with me and ask them to provide you with your benefits (deductible, co/pay, total out of pocket expenses, etc.) Inform your insurance company that I am an out-of-network provider. The insurance company will pay a different amount for an in-network provider vs. and out-of-network provider.
2. At the end of each counseling session you will the self pay rate.. I will give you a “superbill” that provides you all the information needed to file with your insurance company. This can be accessed through your Simple Practice Portal at any time.
3. You then file the “superbill” with your insurance company and they will reimburse you the co/pay based on your benefits. It’s a good idea to send a copy of your insurance card front and back in with your claim.
I suggest you contact your insurance company before submitting your first claim and tell them you are about to send in your health insurance claim form (if they require a claim form). Review all the paperwork you have with them and ask if there is anything else you need. Also, ask your insurance company how long should you expect to wait for your claim to be paid and mark that date on your calendar.
Once you have everything in order, send out the claim form to your insurance company. The address to send the claim form should be on the claim form itself. Keep an eye out on your calendar for the claim date that you marked and contact your insurance company if you don’t receive your claim within the time frame given to you.
4. If your insurance company requires you to mail in an original copy of their own insurance claim form, fill in that form, sign it, and then attach my superbill. Otherwise, just mail in the superbill to your insurance company. Make sure your insurance company’s address is current. If in doubt, call them.
5. Keep a copy of the superbill and any other documentation you submit. Make sure you receive a response for each claim mailed in. Your insurance company may respond to you within two to six weeks. If you have not received a response after 6 weeks, CALL your insurance company to make sure they received the claim. This is very important
6. Many individuals like to submit a monthly superbill, so it is less paperwork. This can easily be provided. Please let me know if you have any questions about this process!
